About this position

The Social Worker provides support and counseling to patients and families in dealing with the impact of illness, disability and episodes of care during hospitalization. Assists the health team in understanding the significance of social, economic, emotional and family factors in relation to the patient's illness, treatment, recovery or dying processes. Facilitates psychosocial assessment, support and advocacy to facilitate positive coping, counseling, crisis intervention, case coordination, education, discharge planning and information and referral. Anticipates potential barriers to discharge planning and consults with the health care team to problem solve and makes referrals to community sources. Assists the health care team in understanding the significance of psychosocial factors in relation to the patient's response to illness, injury or hospitalization. Actively and consistently demonstrates the mission, vision, values and guiding principles of Maple Grove Hospital.

 

What we’ll expect from you

1. Performs an assessment and comprehensive psychosocial evaluation of patient including risk factors of prior functioning levels, adequacy of support systems, reaction to illness and ability to cope including cultural, religious and ethnic background. Develops and implements a comprehensive psychosocial treatment plan.
2. Provides intervention in child abuse/neglect, domestic violence, guardianship, foster care, adoption, mental health placement, adult/elderly abuse, child protection and sexual assault. Completes mandated reporting and assists with petition for commitment.
3. Develops and implements a comprehensive discharge plan in collaboration with patients, their families, multidisciplinary staff and referral agencies which identifies needs and preferences.
4. Assists patients/families in setting up post-hospital placements including TCU/SNF/LTAC, acute inpatient rehabilitation, home care services and DME. Makes referrals to appropriate community services. Participates and involves patient and family in care conferences to identify and interpret care plans, and report concerns as appropriate. Provides education to the patient/family to prepare them for the transition to post-hospital services.
5. Serves as a resource person with regards to emotional, social, and financial consequences of illness and/or disability to meet identified needs.
6. Communicates and collaborates with multidisciplinary team to achieve targeted outcomes. Participates in interdisciplinary care rounds.
7. Provides support and resources related to end of life issues.
8. Maintains knowledge of relevant payer requirements, and services related to payer sources. Partners with case managers, physicians and other healthcare professionals to address issues and clinical appropriateness of admission and continued stay.
9. Recognizes and complies with legal regulatory, accrediting and procedural requirements related to area of responsibility.
10. Understands and follows principles and standards as outlined in Maple Grove Hospital's Code of Conduct.
11. Protects customer and team member privacy and only accesses customer and/or team member related information as needed to perform job duties.
12. Reports violations or areas of concern to leader or Corporate Compliance Officer via established methods of communication.

Education

• Bachelor's degree from an accredited school of social work.
• Masters or Doctoral degree from an accredited school of social work preferred.   

Experience, Knowledge, Skills, Abilities 

• Three + years of Acute Care and Medical/Surgical social services experience preferred.
• Interest in Family Birth Center population preferred.
• Ability to take initiative, be self-motivated and an independent, accountable, dependable performer.
• Solid PC skills to include MS applications, Internet, EMR documentation, etc.
• Strong communication skills with demonstrated ability to express ideas and information (both in written and oral form), clearly and concisely in a manner appropriate to the audience.
• Demonstrated ability to deal with conflict in a positive manner. Has an awareness of, responds to, and considers the needs, feelings and capabilities of others.
• Ability to be accountable for quality customer service and respond appropriately to a variety of personalities and situations.
• Ability to maintain quality, safety, and infection control standards.
• Must be able to read, write and speak effectively in English.      

Licensure/Certification(s) Mandatory

• Licensed as a Social Worker in the State of Minnesota required.
• Minnesota Licensure levels accepted: LSW, LGSW, LISW and LICSW
